Community Health Choices
It is important that you understand the process of Community Health Choices (CHC).
Join us for a (free) meeting at ACHIEVA on Wednesday, October 26, 2016 at 6:30 p.m., 711 Bingham Street, South Side Pittsburgh, 15203 (ACHIEVA building is accessible and parking is free) or via webinar from your home computer for more information on what CHC will look like. Community HealthChoices (CHC) is a new initiative in Pennsylvania that initially will impact older Pennsylvanians receiving long-term services and supports (LTSS) that are paid for by the state and Pennsylvanians receiving both Medicare and Medicaid. The first phase for implementation will begin in Southwestern Pennsylvania on July 1, 2017.
It is a major change for how the state pays for long term services and supports in nursing homes, in the community for older adults and for those with physical disabilities. Currently all of Pennsylvania's waivers will be affected by Community HealthChoices except the Intellectual Disability Waivers (Consolidate and PFD) and OBRA waiver for persons under 21 and 21 years and older who meet the intermediate care facility/other related conditions level of care.
It is important that you understand the process of CHC. In the future, Pennsylvania's ID (Consolidated and PFD) and OBRA waivers may be included in CHC programs. Your child's waiver may look different once the ID Waivers are enrolled in the CHC program.
The three insurance plans that have been selected to manage the Community HealthChoices (CHC) program are:
- AmeriHealth Caritas, a majority owned subsidiary of Independence Blue Cross
- Pennsylvania Health and Wellness, a unit of publicly traded Centene Corp
- UPMC for You, a subsidiary of the University of Pittsburgh Medical Center
